What the form asks for

About the child

  • · Full name exactly as it appears on the birth certificate
  • · Date of birth, and the grade being applied for
  • · Gender, blood group and any medical condition the school should know about
  • · Mother tongue and languages spoken at home
  • · Recent passport-size photograph

About the family

  • · Father's and mother's full names, qualifications and occupations
  • · Primary contact number and email for admission updates
  • · Residential address with pincode, plus a second correspondence address if different
  • · Sibling currently studying at the school, or alumni parent, where the school gives weight to it

Previous schooling

  • · Name and address of the current or last school attended
  • · Grade completed and the medium of instruction
  • · Board followed (CBSE, ICSE, IB, IGCSE or a state board)
  • · Reason for the transfer, when applying mid-school

Document checklist

  • Birth certificate (municipal, matching the name on the form)
  • Two to four recent passport-size photographs of the child
  • Parents' address proof — Aadhaar, passport, utility bill or rent agreement
  • Parents' photo ID and, where asked, PAN
  • Previous school report card for the last completed year (grade 2 and above)
  • Transfer certificate from the previous school, usually submitted after selection
  • Caste, EWS or disability certificate when applying under a reserved category
  • Aadhaar of the child, where the school or the state portal requires it

Filling it in, step by step

    Check the age cut-off

    Confirm the child's date of birth falls inside the school's cut-off for the grade you want. Age is the reason most forms are rejected.

    Collect the documents

    Assemble the birth certificate, address and ID proof, photographs, and the last report card if the child has been to school.

    Register and pay the form fee

    Register on the school's own admission page or collect the printed form at the office, then pay the non-refundable form fee the school states.

    Fill and verify

    Complete every field, match the address to your proof, and re-read the child's name and date of birth against the certificate before submitting.

    Submit inside the window

    Submit before the school's stated deadline and keep the acknowledgement or registration number.

    Attend the interaction

    Attend the parent interaction or observation session if the school runs one, then track the result on the school's list.

Why forms get rejected

  • · The child's age falls outside the school's cut-off for the grade applied for — the single most common reason.
  • · The form was submitted after the school's stated window closed.
  • · Address proof does not match the address written on the form.
  • · A mandatory field or document was left blank or unreadable.
  • · The same child was submitted twice, or details differ between the online and printed copy.
  • · The reserved-category certificate was issued in a format the school or state does not accept.
